£90 (home visit)/ £70 (Consultation room) A focused movement screening and functional assessment, building on your telephone consultation. This is delivered at your home or my west London consultation room in Acton. You’ll receive: • A written summary and a clear, including your personalised self-management plan. • A PDF of your rehabilitation exercises (with progression levels) to guide your plan. Important Booking Information: - In-Person Assessments cannot be booked directly. All new clients must first complete a Step 1: Telephone Consultation before moving to Step 2. - Once you’ve completed your telephone consultation, I will contact to arrange your In-Person Assessment if required. Availability: Home visits: Available in West London within approximately 3 miles of Acton (W3), including: W3, W4, W5, and NW10. Consultation Room (Acton, West London): Appointments are offered on selected weekday evenings and weekends. Exact times will be confirmed when arranging your appointment. I can only see clients who are not registered with GP practices in the NHS boroughs I currently work in.
